Poi - Community of Care

Poi is a new service offered by the Hospices of Auckland. This service focuses on working alongside general practice and residential care providers, to deliver the best possible quality of life for patients and their family/whānau during a person’s final months of life.

Poi services is to enable and guide staff members to identify people’s palliative needs. A personalised palliative care plan is carefully developed with the patient and their family/whānau. With this plan, the patient’s palliative care needs are managed and coordinated collaboratively with general practice, residential care and the Hospice team.
